The former is odd, usually it retains which spells were cast as cast for me. Try resting and studying / praying to see if that resets it properly.
The Silverymoon mythal effects seem to cling if a DM ports you from inside the city to somewhere outside the city. Might fix if you go in and then back out, or on server reset.

A few days ago i was heading to RWK from HH. Just about an inch from the RWK AT leaving the OLM i hit an encounter area with orcs in it... the only trouble is that it was (iirc) the 5c Moonwood encounter area, NOT the Meteor Lake area or anything near there. When I got back out, i was on the OLM up above QV.

This reminds me, the other day along the road to Hawk's Nest it said I found some goblins on the road and that I moved into position, but then nothing happened; no port, no further messages, and I was able to continue along the road.

This has happened to me a few times. What I think is happening is that it is a recent previous encounter that someone balked from facing, and the encounter is left in the screen until taken care of, but I could well be wrong on that account.

you sure it's the rapier and not the fact that you logged off while in subdual mode? doing so will attach permanent -5 penalty to weapon you had equipped...
No idea.
I didn't realize that Subdual Mode was operating differently in NWN2. That's quite the bug!
I'll try Curmudgeon's suggestion of dropping the rapier and picking it back up again. Hopefully that sorts out the problem.

From my experience, dropping it had no effect. The items were bugged permanantly and had to be replaced. Very annoying in the middle of a dungeon crawl with a tank who's main weapon and STR both were creamed by a combination of a bug and a nasty poison trap, I can assure you. In any case, I hope it works for you. Still, a simple rapier is much less of a problem to replace out of pocket than a +1 flaming bastardsword.

The passage is not walkable, so there is no danger of someone thinking that ninjas are jumping out of super secret passages to murder them; it's only confuzing to someone who gets turned around inside said keep.
