Arm wrestling?

If getting all numeric, start throwing in circumstance bonus / penalty for different size categories, maybe +4 per step. End up with a titan arm wrestling a pixie and the conclusion being completely foregone (absent a well placed sleep poison arrow).

EDIT: Don't grappling rules cover this, except skip all the initial steps about initiation of a grapple or even breaking free of a grapple? I.e., just worry about inflicting damage on each other and whatnot, consider it a consensual grapple which neither party will tap out of unless necessary, so two 18 STR fighters, one who has 10 hp and one who has 100hp, odds are the 100hp guy will outlast all the pain n' suffering? Even covers that M critters do 1d3 nonlethal, small ones (hin!) do 1d2... anyhoo, link:
