Finally figured out my issue - AMD R7 conjoined with the Intel embedded card. Seems to work if I move one of my monitors to the motherboard, and disconnect the rest, but then it won't load the HAKs properly. I finally used that to create an alfa.mod, then I can blindly load that in some weird offscreen invisible 4th monitor ..... Grrrr that's hack, not HAK.
Anyhoo - do you still have instructions for the order for adding the HAKs (or a link to an ALFA.mod sample?)
